3.25 or 5.25 contact hours will be awarded to participants who attend the entire program and submit a completed evaluation form. See brochure for details of contact hours of each program. Louisiana State Nurses Association, an accredited approver by the American Nurses Credentialing Center

Registration, Fees and Cancellation Policy
Registration fee includes refreshments, contact hours, and packet of information. Lunch is on your own for the 9/18/08 program. Make checks payable to SELAHEC. Mail check and registration form to Marie Billings, SLU 10835, Hammond , LA 70402

³Unpaid pre-registration prior to the conference date $55.00
³ Registration at the door - $60.00
3.25 contact hour programs
³ Paid pre-registration prior to the conference date $25.00
³Unpaid pre-registration prior to the conference date $30.00
³ Registration at the door - $40.00
The Co-Sponsors reserve the right to cancel the activity if registrations do not meet enrollment criteria. Confirm that conference will be conducted by calling Marie Billings
A full refund will be granted to participants who give a written notice of cancellation of a registration prior to the scheduled date of the conference. Fees may be transferred to another participant or activity during the conference series if preferred.
